For Medical Professionals
Following a successful pilot program in 2025 we are now accepting referrals into our new Meals to Heal program. If you have patients living in West Hants, who are having trouble managing their medical condition or accessing or preparing the food they require for their health, please consider referring them to our program.
If accepted, they will receive meals delivered to their home each week, for at least a six month period. All meals are designed to be nutritionally appropriate for the community member’s health condition(s) and our meals and menus have oversight by a registered dietitian.
Participants receive up to six evening meals a week, delivered frozen plus a selection of healthy snacks.
If you have a client who is interested in the program, they will need to complete pages 1 and 2 of our application form and you, as their medical provider, should complete and sign page 3. Any medical provider with access to the patient’s medical history can complete the application form. Referrals can be made by physicians, nurse practitioners, continuing care coordinators, pharmacists, social workers and dietitians.
Please email the completed forms to us via a secure file transfer system or contact us to arrange for the form to be collected from your practice.
Our meals are prepared by trained chefs in a certified local commercial kitchen (The Station Food Hub in Newport Station) with recipe oversight by a registered dietitian, to ensure meals are suitable for the specific medical conditions. The information you provide on the application form will help us to allocate the appropriate diet for the participant.

Who Can We Accommodate?
For the program, we can accommodate those who have primary conditions of diabetes, heart disease, frailty, cancer treatment or stage 1 or 2 kidney disease and who are finding it difficult to manage their health conditions or to access the food they need to manage their health.
As part of our program, we will conduct an evaluation to help us measure the impact of the program – this will take the form of questionnaires and interviews with participants. As the participant’s medical professional, we plan to contact you after they have received meals for six months, to determine if you have noted any benefits associated with the meals.
